If you forgot your password, here’s a complete guide on how to reset your OSF MyChart account password online;

Step 1: Go to the OSF MyChart Login page at (https://www.osfmychart.org/osfmychart/Authentication/Login). Click on Forgot Password? link below the sign in field.

Step 2: The Forgot Password? link opens the OSF MyChart Password Reset page. Fill the form to verify your personal information.

Step 3: Enter Your OSF MyChart Username in the space provided. If you don’t remember it, you can click on “Forgot Username?” link to recover it and have it sent to the email attached to your account.

Step 4: Input Other Personal Information like your Date Of Birth. It has to follow this sequence; (Month / Day / Year).

Step 5: Click on “Next“, this allows the system to receive your request to reset your password and sends further instructions in setting up a new password.

Step 6: Set Up a New password by entering it with combination of letters and symbols. When done, cross check and verify your new password.
